Every run begins with a choice that captures the game’s split personality: the Lamb you play has just slaughtered dungeon enemies and returns home to find a follower starving, another spreading dissent, and a third demanding a sermon. Which problem you solve first, and how, shapes the cult you’ll have when the next crusade opens. This tension between combat expedition and settlement management is what Massive Monster built Cult of the Lamb around when it released on August 11, 2022.
Combat runs as a top-down action game across procedurally generated dungeon floors organized into four distinct regions, each ruled by a bishop of the Old Faith. The Lamb fights with melee weapons and curse abilities, collecting tarot cards mid-run to modify damage output, movement, and curse strength. Runs end either in defeat or by clearing a region’s boss, after which the Lamb returns to the cult with resources, new doctrines, and followers. Death does not collapse the settlement, but it does cut the resource haul short.
The settlement side operates on a daily cycle. Followers eat, sleep, get sick, age, and die unless the player manages food supply, sanitation, and faith through buildings, rituals, and sermons. Doctrines — unlocked by collecting Commandment Stones — let the player choose between competing cult laws that affect follower behavior and available rituals. Follower loyalty feeds into a leveling system; high-loyalty followers can be designated Disciples, unlocking deeper progression. The tension is resource-based: wood, stone, and gold flow from dungeons into buildings that automate farming, cleaning, and storage, but automation is limited and early cult life requires micromanagement.
The game’s update arc is substantial. Post-launch patches gave way to the free Sins of the Flesh expansion, which introduced follower trait depth, a Disciple leveling system, sinful doctrine paths, quality-of-life automation upgrades including broom leveling and a new Janitor Station tier, and the Blunderbuss as a new combat weapon. The Unholy Alliance expansion added full local co-op with a second playable character, the Goat, alongside new relics, fleeces, follower traits, tarot cards, and co-op-specific mechanics like synchronized attack bonuses. In January 2026, the Woolhaven story expansion launched with its own ranching systems and new content, followed by a rapid series of patches through early 2026. A Cult of the Lamb: Design Works art book was announced for pre-order in April 2026, suggesting the property has grown beyond the game itself.
A new player’s first hour is spent clearing the earliest dungeon region while simultaneously learning that the cult back home is falling apart faster than expected, and that fixing it requires going back into the dungeons for more.
